Visitation Fact Sheet

NYSHFA | NYSCAL has developed a one-page fact sheet for visitation as a template to assist facilities sharing the information with families and friends to better understand each facility’s limited visitation. This sheet is based on the criteria set forth by the NYS DOH July 10th guidance. This document is in word format so you can add your facility name and individualize the sheet to include your facilities’ policies. See attached.

REMINDER- Weekly Nursing Home and Adult Care Facility Staff Testing Survey- Week #9 (opens Wednesday, 7/15 at 9 a.m.)

Please read all of the comments below before completing your survey:

The survey will close on Thursday, 7/16/20 at 5pm. No extensions can be granted.

The survey is unchanged from last week; hence, there is no webinar to review the survey

If you submit your survey and do not receive a confirmation email, then the system did not log your submission

Tip: Please print your responses before submitting (helps with your records and in case you need to resubmit the information)

Refresh the whole webpage and re-enter the survey information you printed out before you resubmit (don't just keep clicking the submit button hoping for a different outcome)

If problems persist, email DOH ASAP (contact info below)

Only submissions that are submitted electronically through the survey system are considered compliant

If a facility does not complete or submit its responses through the electronic survey tool, the facility will not receive NYS kits or PPE supplies for the upcoming week

If you have any questions, the DOH team stands ready to help. Please email either covidadultcareinfo@health.ny.gov or covidnursinghomeinfo@health.ny.gov as soon as a question or an issue appears, so that we can troubleshoot the problem together.
