A mandatory webinar for all Nursing Homes and Adult Care Facilities was held today by the NYS DOH. The webinar covered how to respond to the revised HERDS COVID-19 survey. This survey will be reopened today at 2:00 pm for revisions.

As you answer the survey, complete each box with the items listed below:

  • Contact information must be completed and must include who is the contact person that can be reached day and evening at the facility if questions arise
  • The total census in your building on this date.
  • Number of residents in your facility with lab confirmed positive COVID-19, today
  • Number of residents in your facility with presumed COVID-19 cases, diagnosed by physician today  
  • Number of COVID-19 deaths in your facility since the last census collection (not in hospital)
  • Number of COVID -19 positive deaths that occurred outside in your facility since last census collection
  • Number of residents on isolation in house today (to include all isolations, even if not related to COVID-19)
  • When entering the individual resident(s) deaths, complete each field indicated for each resident who has died:
    • Use initials, comorbidities, date of death, race and gender, and save.  
  • Record all deaths since March 1, 2020 to present day on April 17,2020 for residents who tested positive for COVID or presumed positive for COVID (confirmed by the physician), for residents who died either in or outside of the facility. After April 17, 2020, resume your daily listing of residents for the current date
  • Determine the supply and “burn” rate for PPE and supplies such as: N-95 respirators, surgical masks, gowns, face shield, gloves and hand sanitizer. 
    • Include the donations, purchases and any requested and received supplies from the OEM
