DOH Health Advisory: Protocols for Personnel to Return to Work Following COVID-19 Exposure

NYSHFA/NYSCAL received the information below from the DOH regarding protocols for protocols for personnel to return to work following COVID-19 exposure:


DATE:           March 16, 2020

TO:               Hospitals, Nursing Homes (NHs) and Adult Care Facilities (ACFs)

FROM:          NYSDOH Bureau of Healthcare Associated Infections (BHAI) 

Please distribute immediately to:

Administrators, Infection Preventionists, Hospital Epidemiologists, Medical Directors, Nursing Directors, Risk Managers, and Public Affairs. 

Healthcare facilities may allow healthcare personnel (HCP) exposed to or recovering from Covid-19, whether direct care providers or other facility staff, to work under the following conditions:

  1. Furloughing such staff would result in staff shortages that would adversely impact the operation of the facility.
  2. HCP who have been contacts to confirmed or suspected cases are asymptomatic.  HCP with confirmed or suspected Covid-19 have maintained isolation for at least 7 days after illness onset and have been at least 72 hours fever-free with other symptoms improving.
  3. HCP who are asymptomatic contacts of confirmed or suspected cases should self-monitor twice a day (temperature, symptoms), and undergo temperature monitoring and symptom checks at the beginning of each shift and at least every 12 hours. At the current time, staff who are recovered from Covid-19 should wear a facemask until 14 days after onset of illness if mild symptoms persist but are improving. Staff who are asymptomatic contacts should wear a facemask while working until 14 days after the last high-risk exposure.  
  4. At the current time, staff working under these conditions should preferentially be assigned to patients at lower risk (e.g. on units established for patients with confirmed Covid-19) as opposed to higher-risk patients (e.g. severely immunocompromised, elderly). As this pandemic grows, all staff will need to be assigned to treat all patients regardless of risk level.
  5. If staff who are asymptomatic contacts working under these conditions develop symptoms consistent with Covid-19, they should immediately stop work and isolate at home.  Testing should be prioritized for hospitalized health care workers.  All staff with symptoms consistent with Covid-19 should be dealt with as if they have this infection regardless of the availability of test results.

DOH: Healthcare Personnel Influenza Vaccination Report

The Healthcare Personnel Influenza Vaccination Report will open on Thursday, March 21, 2019 and must be electronically submitted to the New York State Department of Health (NYSDOH) by May 1, 2019. All facilities covered under New York Codes, Rules and Regulations, Title 10, Section 2.59 (10 NYCRR Section 2.59) are required to document the number and percentage of personnel vaccinated against influenza for the current season and to complete this report upon request of the NYSDOH. Alert: Major Winter Storm - Guidance for Hospitals, Nursing Homes, Homecare Agencies, Hospice, Outpatient and Adult Care Facilities

The first of two storm systems will spread across New York State beginning on Thursday evening, with two to five inches of snow expected through Upstate. Over the weekend, New Yorkers can expect to experience 12 to 24 inches of snow during a second storm system. The Governor yesterday activated the State Emergency Operations Center to enhanced monitoring mode, and assets are prepared to be deployed to keep roads and communities safe. DOH: Commissioners Blanket Waiver Letter for Nursing Homes and Residential Health Care Facilities

Please see the attached important letter from the NYS Commissioner of Health to practitioners and pharmacists regarding additional exceptions to the electronic prescribing mandate for nursing homes and residential health care facilities. This letter extends the waiver until October 31, 2019. 

This letter will be emailed to all Nursing Home facilities and pharmacists through the Health Commerce System and will be posted on the Bureau of Narcotic Enforcement’s website in the near future.

URGENT NOTICE ABOUT MODERN DIAGNOSTIC LABORATORY'S SERVICES EFFECTIVE IMMEDIATELY

It has come to our attention today that the Department of Health has suspended Modern Diagnostic Laboratory's services effective May 23, 2018 until further notice. 

NYSHFA staff have had discussions with the highest levels at DOH to highlight the complex issues generated from this suspension of service and its impact on Skilled Nursing and Assisted Living residents. These include the lack of phlebotomists to draw blood, the need for stat blood results, the lack of ability to contract with another lab on such short notice before a holiday weekend and the lack of ability for other labs to adjust their resources to meet this sudden increased demand. NYSHFA will continue to assist DOH as they work towards solutions to maintain resident services. We will continue to update members as more information becomes available.
